Employee Health Benefit Plan
Three (3) Health Benefit options, each at a nominal cost, for eligible staff members to choose from:
- Traditional Plan – includes co-pay & 80/20
- Health Savings Accounts (H.S.A.) - Two (2) H.S.A. Contribution Levels to choose from
Life Policy
A $50,000 Life Insurance policy is included in the Health Benefit Plan for eligible staff members
Prescription Plan
Available to eligible staff members who choose Traditional Health Benefit Plan - Employee pays a percentage of Prescription Cost – No deductible
Supplemental Insurance Policies
Eligible employees may purchase, through payroll deduction, additional insurance policies (Cancer, Accident & Disability, Intensive Care, etc.) through a supplemental carrier.
Dental / Vision
Dental / Vision coverage for eligible staff members and their dependents is available for a nominal premium cost each month
Paid Time Off (PTO)
PTO is based on the number of hours worked
- Full-time (40 hour per week) staff members will earn approximately 168 PTO hours (21 days) annually
- Part-time (16 – 36 hours per week) will earn 80% of the Full-time rate
- Accrual rates increase based on years of service
Bereavement
Three shifts are granted for the death of an immediate family member as defined in policy
Personal Leave
Unpaid Personal Leave (30 days maximum) to Full-time employees to fulfill personal obligations may be granted in the event employee is not eligible for FMLA
FMLA
Family Medical Leave Act – Up to 12 weeks of leave as required by Law.

401(K) Retirement Plan
Full time and part time employees have the opportunity after 1 year of employment to set aside tax deferred dollars through payroll deduction, per federal guidelines, for a retirement savings through Annuities and/or Mutual Fund Investments. Employees are fully vested after 6 years.
Direct Deposit
SMMC requires Direct Deposit. Employees are paid every two weeks, pay periods run from Sunday through Saturday and paid on the following Friday.
125 “Cafeteria” Plan
Pre-tax benefits, (i.e. payroll deductions for daycare, dependent care, orthodontia, out-of-pocket / deductibles / co-pays for major medical expenses) are available to eligible staff members.
